Homes Forever Permanent Supportive Housing
Basic Goals
· To help program participants obtain and remain in permanent housing.
· To help participants increase skills and or income to live as independently as possible.
· To help participant achieve greater self-sufficiency.
Participants will complete an Individual Service Plan (ISP) upon entry and quarterly there after.
Participants will review and revise, with case manager’s assistance their ISP after one year into the program and as needed after.
Requirements of Clients while in the program are to sign and follow lease agreement with their landlord.
- To allow Homes Forever staff access to their apartment or house in situations of health or safety, or program monitoring meetings.
- To respect and care for the apartment or house and to abide by the terms set forth in their lease with the apartment or house Landlord.
- To never allow roommates or overnight guests.
- To report changes in their income to the case manager so steps to self-sufficiency can be reached.
- To allow Homes Forever staff to complete a housing assessment prior to occupying the apartment and then annually. If the apartment or house does not pass inspection, Housing Partners of Western Nebraska Homes Forever Permanent Housing will not provide rental assistance.
- To not have possession of drugs, drug paraphernalia, or other illegal items in the apartment or house or person at any time.
- To establish and follow an Individual Service Plan (ISP)
